If you were seen at a Planned Parenthood affiliate in another state, please contact your local health center to request your medical records.

There are a few ways to request your medical records from Planned Parenthood of Northern New England (PPNNE):

  • MyChart - The easiest way is through our patient portal MyChart. If you already have a PPNNE MyChart account, login and send us a private message to request your records. If you need your records sent to another provider, please provide their contact info in your message. Please note, we cannot set you up with a MyChart account until your next appointment.
  • Fill out a Form
    • If you would like a copy of your medical records for yourselfplease complete this release. Please make sure you have completed all sections of the release.
    • If you would like your medical records sent to another provider, please complete this release. Please make sure you have completed all sections of the release and include provider and practice name, address, and phone number.

How to return the release: Fax it to 802-448-9714. You can also mail it or drop it off  to the Planned Parenthood health center nearest you. Please note that for your privacy, we cannot accept copies of this form through email as it is not a secure method of communication.

Receiving your records: When they are ready, we can either mail your records to you or we can arrange for you to pick them up at one of our health centers. Please note that for your privacy, we cannot email your medical record to you as it is not a secure method of communication.
