New Patient Portal, MyChart, Arrives Nov. 1, 2023

From now through Oct. 31, 2023, you can use our existing patient portal. On Nov. 1, 2023, we are moving to a new patient portal called MyChart. You will not be able to access the old patient portal starting Nov. 1, 2023. To access MyChart, the new patient portal, you will need an access code. When you make an appointment on or after Nov. 1, 2023, you will be emailed the access code to sign up for MyChart. 

Download and save a copy of all the documents in your Patient Portal before 10/31/2023. You will no longer be able to access that information after 10/31/2023.

How to Login to Patient Portal:

Click on this link to log into the system from now until Nov. 1, 2023.

The Portal allows you to:

•    Access your own medical records, patient education, and lab results.
•    Message our Case Management team about lab results questions.

Please request access to your Patient Portal account at your next appointment.

If you still do not see your results after 7-10 business days, login to your account and send us a message. Case Management will respond to your message within one week.
