To ensure the best experience possible, we have provided a list of instructions to help you prepare for your visit.

How to Prepare for Your Appointment:

Annual Exam

Please bring your photo ID, insurance card, copay, policyholder information (including full name and date of birth), and if you have it, an additional card you need to bring with you to the pharmacy to get medications (PBM card). If you do not qualify for funding or if you are not using insurance, payment will be due at the time of service.

We are unable to provide childcare at our health centers. For safety reasons, children are only allowed in waiting room areas, and must be accompanied by an adult (who is not the patient) at all times. Please bring a friend or family member to be with your children if you don't have childcare at home.

For best pap results, do not douche or place anything in the vagina 24 hours before the appointment. If you use the Nuvaring, you can continue using this as normal.

If you have your menstrual cycle and are spotting or bleeding lightly, we can continue with your appointment. Otherwise, please reschedule your appointment for another time.

If you need to cancel or reschedule your appointment, please let us know as soon as possible.

Birth Control Visit (Pill/Patch/Ring/Depo)

Please bring your photo ID, insurance card, copay, policyholder information (including full name and date of birth), and if you have it, an additional card you need to bring with you to the pharmacy to get medications (PBM card). If you do not qualify for funding or if you are not using insurance, payment will be due at the time of service.

We are unable to provide childcare at our health centers. For safety reasons, children are only allowed in waiting room areas, and must be accompanied by an adult (who is not the patient) at all times. Please bring a friend or family member to be with your children if you don't have childcare at home.

Nexplanon (insertion)

Please bring your photo ID, insurance card, copay, policyholder information (including full name and date of birth), and if you have it, an additional card you need to bring with you to the pharmacy to get medications (PBM card). If you do not qualify for funding or if you are not using insurance, payment will be due at the time of service.

We are unable to provide childcare at our health centers. For safety reasons, children are only allowed in waiting room areas, and must be accompanied by an adult (who is not the patient) at all times. Please bring a friend or family member to be with your children if you don't have childcare at home.

If you are not using hormonal birth control, we recommend you abstain from vaginal intercourse for 14 days prior to your appointment. If you are using hormonal birth control, please continue taking it normally.

If you need to cancel or reschedule your appointment, please let us know as soon as possible. 

Intrauterine Contraception (IUD or IUC): Mirena or Paragard

Please bring your photo ID, insurance card, copay, policyholder information (including full name and date of birth), and if you have it, an additional card you need to bring with you to the pharmacy to get medications (PBM card). If you do not qualify for funding or if you are not using insurance, payment will be due at the time of service.

We are unable to provide childcare at our health centers. For safety reasons, children are only allowed in waiting room areas, and must be accompanied by an adult (who is not the patient) at all times. Please bring a friend or family member to be with your children if you don't have childcare at home.

If you are not using hormonal birth control, you must abstain from vaginal intercourse for 14 days prior to your appointment. If you are using hormonal birth control, please continue taking it normally.

Please eat something and take 800mg of ibuprofen one hour prior to your appointment.

If you need to cancel or reschedule your appointment, please let us know as soon as possible.

Pregnancy Test

Please bring your photo ID, insurance card, copay, policyholder information (including full name and date of birth), and if you have it, an additional card you need to bring with you to the pharmacy to get medications (PBM card). If you do not qualify for funding or if you are not using insurance, payment will be due at the time of service.

STI Testing (With or Without Symptoms)

Please bring your photo ID, insurance card, copay, policyholder information (including full name and date of birth), and if you have it, an additional card you need to bring with you to the pharmacy to get medications (PBM card). If you do not qualify for funding or if you are not using insurance, payment will be due at the time of service.

A routine screening includes tests for the most common Sexually Transmitted Infections (STIs), including: Chlamydia, Gonorrhea, and HIV. A test for Chlamydia and Gonorrhea should be done two weeks after possible exposure. HIV testing should be done 90 days after possible exposure. We offer rapid HIV testing which is done through a small sample of blood, with results in 10 minutes.

For most accurate urine testing, please do not urinate for one hour before your visit. Female patients: Chlamydia and Gonorrhea testing can be performed by a vaginal swab. If you are showing symptoms, do not douche or place anything in your vagina for 24 hours prior to the appointment.

Urinary Tract Infection (UTI)

If you do not qualify for funding or if you are not using insurance, payment will be due at the time of service.

For most accurate testing, please try not to urinate for one hour before your visit.
