As a patient of Planned Parenthood of Greater Ohio, you have a right to:

  • Access the information Planned Parenthood of Greater Ohio keeps about you.  This may include records related to your visits and procedures, test results, and doctors’ notes.
  • Tell us to send your information to others, if we keep that information in electronic form.

Your right to information extends to your medical records, billing records, and other information that Planned Parenthood of Greater Ohio keeps about you for clinical purposes. If you want information about services received from other Planned Parenthood affiliates, you should contact those organizations directly. 

In rare cases, we may deny your request for information.  For example, if we believe supplying you with certain information could lead to physical harm to you or someone else, we will withhold that information.  In some cases, you have a right to appeal our denial.

Completed requests can be submitted through MyChart (our patient portal), or by taking the completed form to your nearest Planned Parenthood health center along with your identification.

Getting Your Health Records

If you are a Planned Parenthood of Greater Ohio patient and would like to request a copy of your health records or have us send your information to another provider, please read the following:

You can access most of your medical information through the Patient Portal. If you are not currently enrolled in our Patient Portal and would like to be, contact our Patient Contact Center at 800-230-PLAN (7526).

If you want to access your information and it is not in the Patient Portal or you do not want to use the Patient Portal, you can let us know by telling us in-person, emailing, or sending us a request in the mail. Please see the next section below to access the request form.

We will try to provide your information in the format and via the method you ask for, but we may have to provide your information in an alternative manner if it is not feasible to follow your request.

We will respond to your request as soon as reasonably possible.
